ADMISSION CRITERIA

 

Individuals are interviewed and reviewed by the Admission Committee of

RISE UP, LLC.  The Admission Committee consists of the CAO, Program Director, and Program Managers.

 

Applicant must be at least 12 and not older than 18.

  • Applicant must have a primary diagnosis of Intellectual Disabilities, developmentally disabilities, or a DSM-V diagnosis.

  • Applicant must not exhibit behaviors consistent with actively abusing drugs or alcohol or must accept treatment if applicant is abusing.

  • Applicant must participate in some form of activity outside the home (day support, school, employment, volunteering)

  • Applicant must be able to be maintained in a group home environment which is the least restrictive environment.

  • Applicant must be willing to participate in the program.

  • Applicant must complete a physical examination with TB Test or screening within 30 days prior to admission date or no later than seven days following the admission Health Information date. Information must be documented on a physical exam form and signed by a licensed physician.

 

EXCLUSION CRITERIA

 

  • Individuals with major medical problems that require continuous medical oversight or treatment such as the provided in an acute care hospital (i.e. 24-hour skilled nursing care, on-site physician, technological monitoring)

  • Individuals who are currently suicidal and are in imminent danger of harming themselves thus requiring acute services or physically secured environment. (i.e. in-patient hospital services or 24–hour 1:1 supervision)

  • Individuals who are currently homicidal and are in imminent danger of harming others thus requiring acute services or physically secured environment. (i.e. in-patient hospital services or 24–hour 1:1 supervision)

  • Individuals who require inpatient medical detoxification for drug or alcohol abuse.

  • Individuals who are actively psychotic

  • Individuals who have a history of fire setting (assessed on an individual basis)

  • Individuals requiring detention services

  • History of running away (assessed on an individual basis)

  • Past or pending felony charges or currently physically violent (assessed on an individual basis)
